At the finish the runway opens into a plaza packed with a crowd: plow through with your now-giant helmet for a live hit count and cash it in for stars.

You run down a construction runway collecting hard hats, and each one makes the helmet on your head bigger. Steer by dragging or with left and right. Brick walls block the runway at intervals, and whether you go through one or bounce off it is decided purely by how big your helmet is when you arrive - so the first half of the run is about arriving at each wall with enough. At the end the runway opens into a plaza full of people and you plough through them with whatever helmet you have built, racking up a live hit count that converts into stars. The run therefore has two distinct halves: accumulate, then cash in.
Helmet Hero is a grow-then-spend runner with an unusually clear structure: the walls turn the accumulation phase into a series of pass/fail checkpoints, so you always know whether you are on pace rather than finding out at the end. That is a small design decision that makes the game much more readable than the genre norm, where players usually cannot tell a good run from a bad one until it is over. The plaza finale exists to make the accumulated size visible - a number on screen is abstract, a crowd scattering is not - and the live hit count during it is the actual score. Construction site, hard hats, comically large helmet: the joke is the mechanic.
